TC2 Accountable Care Organization

At TC2, we believe that healthcare providers should be accountable for the care they deliver. TC2 (Total Care Collaboration) is committed to delivering high-quality collaborative care in a cost-effective manner.

TC2 consists of a network of affiliated physicians and hospitals that unites physicians of every specialty throughout the Middle and South Georgia area in a common commitment to quality and accountability. Our hospital partners include: Coffee Regional Medical Center, Crisp Regional Hospital, Houston Healthcare, and Navicent Health, numerous specialty and advanced inpatient/outpatient care facilities. With Hospitals and Physicians aligned, we strive to deliver better clinical outcomes through cutting-edge technology and evidence-based medicine.

Connecting Providers to Deliver More Coordinated Care

At TC2, we’re advancing health in our local communities by connecting care delivery, via physicians and hospitals through confidential advanced data sharing. Our hospitals and affiliated physicians utilize information technology and specialized tools to increase care effectiveness and safety while getting patients more engaged in managing their health. Through a process called clinical integration, these healthcare providers share information about patient care with each other. This allows them to deliver care that is more coordinated and therefore, more cost-effective. Each healthcare provider treating a patient receives a more complete picture of that patient’s health so physicians and patients can make better, more informed healthcare decisions. In addition to delivering care that leads to shorter hospital stays, TC2 physicians aim to keep patients healthy and out of the hospital in the first place. Through the use of preventive medicine and a team-based, proactive approach, TC2 physicians can identify patients at risk of illness before they become ill. For patients with chronic conditions, TC2 offers a full continuum of care that leads to better clinical outcomes and controls healthcare costs.
